Are you a member of a team taking part in the competition? 

All team members must fill out this form by August 27, 2021 at 9:00a.m. If a team member is under the age of 18, their parent or guardian should fill it out instead. 

Please note: Only the person who submitted the team’s design will be contacted if your team wins.
